I. MATERNITY BENEFITS
     (Rs. 2000/- from 01.10.2008 onwards – 2 times)
a.  Attested copy of the birth certificate of the child, identity card and passbook received from the welfare board.
b.  Application should be submitted within three months.
c.  Minimum one year service as a member compulsory.
d.  Application in the prescribed form.
 II. MARRIAGE BENEFITS
      (Rs. 2000/- from 01.10.2008 onwards -2 times)
a.  Attested copy of the marriage certificate received from the Local self  Government bodies or from the Registration Department.
b.  Attested copy of identity card and passbook received from the welfare board.
c.  Application should be submitted within three months time.
d.  Only Rs. 1000/- is eligible for male members.
e.  Application in the prescribed form.

III. FINANCIAL ASSISTANCE FOR TREATMENT
       (Maximum upto Rs. 5000/- once)
a.  Application in the prescribed form .
b.  Medical treatment certificate received from a physician not below the rank of an Assistant Surgeon in the Government service.
c.  Treatment for non-malignant diseases an amount up to Rs.1000/-
d.  Financial assistance up to Rs.5000/- for the treatment for the diseases like heart problem, kidney problem, cancer, brain tumour,
     dialysis etc. and for various surgery.
e.  Attested copy of ID card and pass book received from the welfare board.
f.  Application should be submitted within three months from the date of discharge.
g.  Copy of discharge certificate.
h.  Three years membership compulsory.

IV. RETIREMENT PENSION
       (Two copies of application along with related documents each )
       (Minimum amount of Rs. 1000/- from 01.06.2016 onwards.)
a.  Pension application in the prescribed form No: 10A
b.  ID card and passbook received from the welfare fund board (Original & Copy)
c.  Joint Photo of husband & wife (member and spouse)
d.  An affidavit showing the retired member is not receiving any similar type of pension from the Government.
e.  Application should be submitted within 90 days from the date of pension.
f.  Minimum three years of service is compulsory as a member.

V. DISABILITY PENSION
      (Two copies of application along with related documents each)
      (Minimum amount of Pension)
a.  Application for pension prepared in form No. 10C
b.  Certificate from the medical board showing the disability.
c.  ID card and passbook received from the Welfare Fund Board(Original & Copy)
d.  An affidavit showing no similar type of is pension receiving from the Government.
e.  A photo taken within 6 months period.

VI. FAMILY PENSION
      (Two Copies of application along with related documents each)
      (Minimum amount of pension)
a.  Pension application prepared in form No.C
b.  An attested copy of the death certificate of the pensioner.
c.  A legalheirship certificate from the Village officers showing the family relationship with the demised pensioner.
d.  Pension book received from the Welfare Fund Board.
e.  A photo taken with in 6 months period.

VII. FUNERAL EXPENSES/EXPENSES FOR FUNERAL RITES
       (Rs.1000/-)
a.  An application submitted by the nominee/ legalheir to the District Officer along with the attested copy of the death certificate of the
     member, original ID card and passbook received from the welfare board.
b.  Prescribed application along with copy in appendix I

VIII. FINANCIAL ASSISTANCE AFTER DEATH
a.  Application prepared for financial Assistance in Form No: 9
b.  Attested copy of death certificate.
c.  Original ID Card and Pass Book of the demised member received from the Welfare Board.
d.  Attested copy of the identity of the legalheir applying for the benefit.

IX. SCHOLARSHIP
a.  Application in the prescribed form attested by the school/college principal.
b.  Attested copy of the mark list of the eligible examination
c.  Attested copy of the Welfare Fund Board ID card, pass book of the parent.
d.  Application should be submitted within three months from the date  of beginning of the course.
e.  One year service compulsory for the members.
f.  An amount of Rs. 600/- to 5000/- will be sanctioned for each courses from each districts to a maximum of two students and who ever
     receives the highest mark for the selected +2, graduate ,post graduate and diploma courses.
X. CASH AWARD
a.  Attested copy of welfare Fund ID Card, pass book along  with the prescribed application form must be submitted within
     three months from the date of result.
b.  One Year service is essential for the members.
c.  Students getting full A+ mark for all subjects in S S L C are eligible for Rs. 1000/- each.

XI. SPECIAL INSTRUCTIONS
1.  Certificates submitted along with the application having difference in name and address with the records of Tailoring Workers Welfare
     Fund records should submit a one and same certificate from the Village officer or a Gazetted officer.
2.  An application submitting after the stipulated time but within six months period should submit a request for condonation in
     delaying submitting the application to the District Executive Officer.
3.  Benefits will be disbursed through bank accounts only.
4.  Memberships will get cancelled if the subscription amount is not remitted within (12 months) or one year period. As per rules the lost
     membership can be revived for not more than two times. The applications submitted during the subscription arrear period are not
     eligible for the benefits.
5.  When members applying for the financial assistance from the Welfare Fund Board should submit the copy of passbook showing
     bank account number and IFSC Code.

REGISTERING IN THE WELFARE FUND BOARD
1. An application should be submitted in form No: I along with the certificate showing proof of age, a certificate received either from
    Assistant Labour Officer, an employer or a registered trade union (Registered asper trade union act 1926) showing the work experience.
2. Rs. 15/- may be remitted on registration fees.
